The community chapter cannot be back-filled the week before a GAHAR survey.
Overview
A quality, safety and community care management system for primary healthcare centres, written to the GAHAR Handbook for Primary Healthcare Accreditation Standards, First Edition 2021, effective February 2021. It maps to all 14 GAHAR chapters, from patient-centredness and integrated care delivery to community assessment and involvement, the CAI chapter most often neglected and able to cause rejection on its own. The quality manual carries the 14-chapter compliance map, the 21 National Safety Requirements and 26 quality indicators.
Primary care breadth is handled programme by programme: immunisation and the vaccine cold chain, child health, maternal health, reproductive health and family planning, non-communicable disease review, oral healthcare and first-visit screening, each with its own procedure, record and audit. Every National Safety Requirement is traced to a specific procedure, covering patient identification, hand hygiene, critical results, surgical safety, cold-chain medication storage, radiation and laboratory safety, and the full set of facility management plans. Disease surveillance line lists, notification records and community health education records close the CAI gap, none of which can be produced retrospectively. Retention is set out in a master records register with the extended periods that child health, immunisation and maternal records require.
What this system covers
- Immunisation and the vaccine cold chain — refrigerator temperature logging, excursion handling and cold-chain audit
- Maternal, child and reproductive health — antenatal records, child health, family planning and programme audit
- Community assessment and involvement — surveillance line lists, notifiable disease reporting and health education records
- Safety requirements in daily practice — patient identification, hand hygiene, critical results, surgical safety and time-out
- Access, flow and continuity — registration, triage, referral and loop closure, care plans and patient education
- Medication and diagnostic safety — reconciliation, administration, storage, radiation safety and laboratory safety
- Survey readiness — scored gap analysis with findings tables and corrective action transfer, worked against GAHAR's four decision rules
Who it's for
Centre managers, quality officers, cold-chain officers, surveillance focal points and accreditation coordinators at primary healthcare centres, family medicine units and polyclinics in Egypt. The purchase usually follows an accreditation date being set while the programme and community chapters still have nothing behind them, and it leaves the centre with a phased implementation plan, a scored baseline and a records register built to survive inspection.
